A good overview of confined space entry, with some omissions and some missing information. It is important to know that working in a confined space will also involve working at height, and dropped objects are also a concern. Consider a lanyard for torches and tools. Usually the confined space is not lit, so our minimum for my company is to have at least two torches, normally a headtorch and a hand-torch (with a lanyard of course). Inside the tanks and void spaces in the Gryphon FPSO there are often lighting holes through which an unaware person could fall. For this reason my recommendation is to look at where you are walking and never carry out any inspections while walking (stop and look). Suspension trauma is actually a concern in that you are wearing a rescue harness, and correct donning of the harness and adjustment to fit could prevent harm when rescuing a person using the harness. On an FPSO there is an additional hazard as the confined spaces can often be in the vicinity of a hydrocarbon plant. For this reason in a tank that is gas free adjacent to the process area there is a requirement to carry an 'oxybox' escape unit with you, If there is a hydrocarbon release which results in gas inside the tank, the personnel in the tank can use the 'oxybox' kit to escape. This is best explained to people used to working in confined spaces in ships. You may find that their company forbids them entry to a space that has not been gas freed, and so they may refuse to enter a tank that requires them to wear an 'oxybox', so they need to have the requirement explained to them, ie space is gas free but adjacent to hydrocarbon plant. Very long course, And most of it can be cooked down to, psychical barrier, watchman, continuing monitoring and proper preparation, anything else doesn’t matter the most. Also I’m very tired of it doesn’t work on IOS
