My address was wrong the Young lady that I spoke with finally got it straight I received it by email instead of regular mail. After that everything was on point waiting on the return from the Health Department.

Tedious; but easiest way to do it online
If you live out of the state you are born with, 10/10 this is the way to go. Gotta pay for the packet and include a 22$ fee in the packet itself when you mail it - otherwise you’re screwed and have to wait even longer. Obtaining official documents is annoying and boring but must be done. A trip to your County’s Clerks office is definitely quicker though but not always an option
