The Check-in Fiasco

Long story short -- I learned that a check-up visit was required to renew my heart medication. I made the appointment upon this discovery, but the check-up was scheduled a month out, and I was out of medicine. It was a simple matter of the doctor writing a temporary fill to get me through to the check-update (about 20 days). But nope. I've made 4 calls to the office, 4 messages through their "Klara" app, and 4 patient portal messages, I am no closer to getting the heart medicine. I was told by a nurse to take the medicine consistently and not to skip doses due to the risk of a stroke. At this point, I am not sure if or when the prescription will be issued. There doesn't seem to be a good process for escalating the urgency. Had I known about the check-in visit I would have made it well in advance.
Even if I was supposed to be aware of it and make the required check-in visit, the response to the mishap is inadequate. I hope I can resolve it before going weeks without the medicine.
