A group of three of us (one USA, 2 UK) just had a WONDERFUL 17 day private escorted tour in Pakistan with "Travel and Culture Services." We organized the entire thing by email, with excellent responsiveness and customer service throughout the entire process of booking, getting visa documentation, etc. The itinerary was fantastic (entire length of the country), the guide and driver were outstanding, and the price was extremely reasonable, and customizable to your wishes (e.g., we wanted 2-3 star hotels, but they can book high end also). I can't recommend these guys highly enough.
