To Whom It May Concern,
I am writing regarding the unauthorized use of my father's Medicare number and several critical compliance and privacy violations committed by your facility.
Specifically, your facility utilized my father's Medicare number to submit billing claims for services that were either never authorized, never rendered, or for which he was not an eligible patient. This constitutes a direct violation of the False Claims Act (31 U.S.C. §§ 3729-3733) and a serious breach of your Medicare Provider Agreement.
Furthermore, accessing, sharing, or processing my father’s Protected Health Information (PHI) without authorization or a legitimate medical/billing purpose is a severe violation of the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules.
Despite multiple attempts on my part to contact representatives at your facility—specifically Christian, Cecilia, and Paulina—to resolve this matter, I have received absolutely no response, explanation, or corrective action regarding this "mistake." This total lack of communication and compliance oversight is unacceptable.
Please be advised of the following required immediate actions:
Provide a complete, written explanation of how and why my father's Medicare number and PHI were accessed and billed. Provide a full ledger of all claims submitted under his Medicare number. Confirm in writing that all fraudulent or erroneous claims have been formally reversed and retracted from the Medicare system. Detail the internal corrective and disciplinary measures taken to ensure this breach does not recur.
I have filed formal complaints with the following: The Department of Health and Human Services (HHS) Office of Inspector General (OIG) for Medicare fraud and False Claims Act violations. The Centers for Medicare & Medicaid Services (CMS) for Provider Agreement violations. The HHS Office for Civil Rights (OCR) for HIPAA privacy violations.
This Company is Amazing the personnel and staff working with and for them are absolute gyms, they're helpful kind considerate and respectful.

contacted Solace and had a conversation with a rep. Then, I was scheduled a very short phone call with a Dr.
I wasn’t aware if my insurance would cover it and I could not afford it, so I told them I wouldn’t be using their services. I was then told by email that I would be charged for the “time” spent on the phone. This cost was never mentioned in any phone calls or emails. They then proceeded to charge my insurance for over $400 dollars for two phone calls???? I was going to go ahead and try them and see if it worked for me before they did this. Now I would not want to deal with their company at all.
Btw, they never contacted me at all!! Even though they say in their reply that they will.

Absolute disaster, I want to know which health insurance company Solace lied to and robbed blind for not providing any service to me. I complained numerous times and absolutely nothing changed at all. The advertising is ambiguous the company does not tell the whole truth about the services provided consequently I believe fraudulently billing for services that did not happen in my case especially. I have complex medical and psychological illnesses. Solace was not able to help me Outside of saying they would find someone else to help me NEVER happened. Also I spoke T length about what I needed and after six months nothing, changed and there were no additional resources brought to my attention from this company. This company does not do what they say at least when I first signed up so therefore that is straight unambiguous fraud, in my opinion. I gave the company ample opportunity to make corrections and provide a service. I’m done with the non sensical foolishness and untrue rhetoric. Now tell me who they billed so that I can let the company paying for the service know that this was a scam in my opinion.
